Notice of Request for Data. Data Recipient agrees to notify EMS Authority within five business days of Data Recipient’s receipt of any request or subpoena for Protected Health Information relating to this Agreement. Such notice shall be in writing and may be sent by fax or email provided that any such notice shall also be sent to EMS Authority by personal delivery, certified mail, return receipt requested, or by nationally recognized overnight courier to the following: EMS Authority Name Department Address City, State, Zip Code Phone Number Fax Number Email To the extent that the EMS Authority decides to assume responsibility for challenging the validity of such request, Data Recipient shall cooperate fully with EMS Authority in any such challenge.
