Common use of OPTAVIA COACH INFORMATION Clause in Contracts

OPTAVIA COACH INFORMATION. Coaches must supply the Company with a valid mailing, e-mail address, and phone number for communication purposes (pursuant to the terms of the Agreement, the Coach consents to being contacted by the Company regarding their OPTAVIA Coach business). Each Coach is responsible for keeping their information (name, address, phone number, e-mail address, etc.) up to date and accurate and must immediately update the Company on any changes concerning this information. The Company may terminate an OPTAVIA Coach business if the Company determines false or inaccurate information was provided by the Coach. If a Coach fails to update their information (including any information on a Business Entity Addendum), holds may be placed on his/her account or other disciplinary action may be taken, up to and including termination. In addition, the Company will not be held responsible for communications and/or information not received by Coaches due to failure to update information on their account or on the Business Entity Addendum.
