Option Plan Description. For 2012-2015, the Board shall provide the following health insurance options for all members of the bargaining unit and their eligible dependents: BCBSM PPO ($100/$200 deductible, $10/$20 prescription co-pay). BCBSM Simply Blue PPO ($1250/$2500 deductible; $5/$25/$50 prescription co- pay; a health savings account (HSA) funded by the College for 100% of the deductible amount). The employee may contribute more. Funds in the HSA may be used to cover deductible expenses and other qualified medical expenses, including prescription drugs. Blue Care Network HMO ($5/$10/$25 co-pays for office visits/urgent care/emergency room; $5/$10 prescription co-pay).
