Optional Coverage. No later than 30 days prior to the date established by the City, an employee in active service or who after that date retires on disability and under the age of 65 eligible for and taking base coverage, shall be eligible to apply for supplemental coverage effective the first day of the next month following the next open enrollment (as determined by the City) for supplemental life insurance at his/her option in increments of $1,000 to a maximum of either
Optional Coverage. The Nurse’s spouse/domestic partner may be enrolled in the plan at group rates, at the option of the Nurse. The spouse/domestic partner must be enrolled within one month of the Nurse’s eligibility or enrollment may be delayed to dates established by the insurance carrier.
Optional Coverage. In addition to the basic coverage, a teacher may apply for additional life coverage in multiples of $10,000 to a maximum of $500,000. Optional coverage is further limited in that the combined total of the basic coverage and the optional coverage for an individual teacher shall not exceed a maximum of $525,000.
Optional Coverage. The following coverages are available at the option of the Customer and cover only items specifically labeled as “Included” and are subject to all other provisions, limitations and exclusions in this Contract. Your optional coverage, if selected, is listed on the Cover Page.
Optional Coverage. Three times (3X) annual salary, the cost of such insurance shall be shared equally by the employee and the Employer.
Optional Coverage. Pays 70% of salary with a $7,000 monthly maximum. Employees pay the difference between the cost of this option and their cost for basic coverage. Compensation under these plans may be reduced by payments from other sources such as Workers’ Compensation and Social Security. The total amount of compensation from all sources under these plans will be 60-70% of monthly income depending on the plan selected. The total monthly benefit will not exceed $6,000 or $7,000 if the optional coverage is in effect. Payments begin on the first of the month after the employee has been totally disabled for six months. After 42 months of continuous disability, monthly payments will be increased 3% annually or by the Consumer Price Index if lower.
Optional Coverage. The purchase of optional coverage is not a requirement to rent a JUCY vehicle. Coverage we sell may duplicate your own personal automobile liability policy or by another source of coverage. We are not qualified to evaluate the extent of your existing coverage. Optional coverage is provided under individual policies issued to you, or issued to you under a group or master policy issued to us by an insurer authorized to transact the applicable coverage in the state of California.
Optional Coverage. Four times (4X) annual salary, the cost of such insurance in excess of b) above shall be paid by the employee.
Optional Coverage. During an annual open enrollment period established by the City, an employee in active service or who after that date retires on disability and under the age of 65 eligible for and taking base coverage, shall be eligible to apply for supplemental coverage, at his/her option in increments of $1,000 to a maximum of 1.5 times his/her annual basic salary rounded to the next higher thousand dollars of earnings.

Optional coverage is further limited in that the combined total of basic coverage and the optional coverage for an individual shall not exceed a maximum of $525,000.
