Oral Surgery Services. Procedures performed by properly licensed Dentists for extractions and other oral surgery, including preoperative and postoperative care.

Oral Surgery Services. Oral Surgery: Extractions and certain other surgical services and associated covered anesthesia and/or related covered services. Extractions —Surgical Extractions of Teeth or Tooth Roots Includes local anesthesia and routine post-operative care. Oral Surgery Services Includes but not limited to fistula closure, sinus perforation closure, tooth reimplantation, surgical access to expose teeth, biopsies, soft-tissue lesion removal, excision of bone tissue, excision of hyperplastic gum tissue, surgical incisions, and cyst removal. Includes local anesthesia and routine post- operative care. Benefits are based on clinical review. Alveoloplasty Included when performed on the same date as extractions and includes local anesthesia and routine post-operative care.
Oral Surgery Services. Extraction – Covered Services include: General Anesthesia, Intravenous Conscious Sedation and IV Sedation – Covered when administered with covered surgical service.
Oral Surgery Services. Extractions and all other oral surgery procedures usually employed by dentists. By way of description, such service includes pre and post- operative care.
Oral Surgery Services. Oral Surgery Services - These services include post-operative care such as examinations, suture removal and treatment of complications • Extractions, including Surgical extractions - Removal of impacted teeth is covered only when evidence of pathology exists • Alveolectomies • Biopsy of oral tissues • Excision of cysts and neoplasms • Frenectomy • Incision and drainage of abscesses • Root recovery (separate procedure) • Treatment of palatal torus and mandibular torus • Basic tooth extractions Benefits include the following: • Crowns – including those made of acrylic, acrylic with metal, porcelain, porcelain with metal, full metal, gold onlay or three-quarter crown, and stainless steel. Related dowel pins and pin build-up are also included. Crowns are covered as followed: o Replacement of each unit is limited to once in a thirty-six (36) month period, except when crown is no longer functional o Only acrylic crowns and stainless steel crowns are covered for children through age eleven (11). If other types of crowns are chosen for children through age eleven (11), it will be considered an optional treatment and the covered dental benefit level would be that of an acrylic crown o Crowns are covered only if there is not enough retentive quality left in the tooth to hold a filling o Veneers posterior to the second bicuspid are considered and optional treatment. We will pay up to the allowance for a cast full crown • Recementation of crowns, inlays, and onlays • Cast post and core, including cast retention under crowns • Crown repair • Repair or replacement of abutments or pontics • Fixed bridges – Bridges made of cast, porcelain baked with metal, or plastic processed to gold are covered as follows: o Members who are age sixteen (16) and older are covered. Fixed bridges for Members under age sixteen

  • Mental Health Services Grantee will receive allocated funding to secure Mental Health Services and Programs for youth under Xxxxxxx’s supervision. Services may include screening, assessment, diagnoses, evaluation, or treatment of youth with Mental Health Needs. The Department’s provision of State Aid Grant Mental Health Services funds shall not be understood to limit the use of other state and local funds for mental health services. State Aid Grant Mental Health Services funds may be used for all mental health services and programs as defined herein, however these funds may not be used to supplant local funds or for unallowable expenditure. Youth served by State Aid Grant Mental Health Services funds must meet the definition of Target Population for Mental Health Services provided in the Contract.
