PAPER APPLICATION. If the applicant cannot get on the FAA network, there is a paper Form 1681 ID Application (attached to the ISD/Final Suitability email) that can be completed. The applicant will complete the top section and hand-carry or scan/email to the PIV Sponsor to have it signed in the center section. The form will then be brought with the applicant to an FAA badging office. Both of these processes require two forms of government issued, non-expired ID to complete the enrollment. A list of these forms of ID can be found at: xxxx://xxxxxxxxx.xxx.xxx/org/staffoffices/ash/id_badge/media/acceptable_PIV_documents.pdf At the time of enrollment a photo and biometrics will be taken and a temporary 30-day yellow badge will be given to the applicant. The permanent badge takes approximately a week to 10 days to arrive. Once the card arrives a final email will be sent to the applicant indicating their card has arrived. The applicant will then proceed back to the FAA badge office where they enrolled and turn in their yellow card so they can Activate their permanent PIV card. Please note that there are THREE steps to the PIV Card - ID Badging process: Application: Completed either online or in paper form (as described above). Must be approved by a PIV Sponsor. Enrollment: You have received an email stating you have been approved and/or have completed a paper 1681 that has been signed by a PIV Sponsor. (NOTE: This is when you will need your TWO forms of ID).
