Application Procedures i) An employee applies for a listing on the system-wide registry through the employee’s Human Resources Department by completing the form in Appendix A.
ii) The institution will immediately forward the completed form to the PSEA who will list eligible employees on the system-wide registry.
iii) A registrant is responsible to ensure the information is current and to immediately notify the Employer and the local Union if the registrant is no longer available for employment through the Registry.
Application Procedures. All applications under this Section 2.2 shall be made in accordance with the procedures adopted by the CAISO from time to time and shall be accompanied by:
(i) a description of the transmission lines and associated facilities that the applicant intends to place under the CAISO’s Operational Control;
(ii) in relation to any such transmission lines and associated facilities that the applicant does not own, a copy of each document setting out the applicant’s Entitlements to such lines and facilities;
(iii) a statement of any Encumbrances to which any of the transmission lines and associated facilities to be placed under the CAISO’s Operational Control are subject, together with any documents creating such Encumbrances and any dispatch protocols to give effect to them, as the CAISO may require;
(iv) a statement that the applicant intends to place under the CAISO’s Operational Control all of the transmission lines and associated facilities referred to in Section 4.1 that it owns or, subject to the treatment of Existing Contracts under Section 16 of the CAISO Tariff, to which it has Entitlements and its reasons for believing that certain lines and facilities do not form part of the applicant's transmission network pursuant to Sections 4.1.1.i and 4.1.1.ii;
(v) a statement of any Local Reliability Criteria to be included as part of the Applicable Reliability Criteria;
(vi) a description of the applicant’s current maintenance practices;
(vii) a list of any temporary waivers that the applicant wishes the CAISO to grant under Section 5.1.6 and the period for which it requires them;
(viii) a copy of the applicant’s proposed TO Tariff, if any, must be filed;
(ix) address and contact names to which notices under this Agreement may be sent pursuant to Section 26.1;
(x) any other information that the CAISO may reasonably require in order to evaluate the applicant’s ability to comply with its obligations as a Participating TO; and
(xi) details of the applicant’s intent to establish a settlement account.
Application Procedures. Application is made by a letter of request to the Curriculum Deans and appropriate planning groups, stating the dates and proposed activities of the requested leave. The request should be submitted twelve (12) months before the start of the academic year in which the leave will take place, or with as much advanced notice as possible. Proposals submitted later than April 1 are less likely to be approved. Emergencies may require later requests, and these shall be given exceptional consideration.

Application Procedures. 15.13.6.1 Applicants for Staff Development Leave shall submit a completed Application for Staff Development Leave Form (Appendix F) to the Immediate Management Supervisor who will forward it through established administrative channels. The application shall be submitted at least thirty (30) calendar days prior to the first date of the requested leave and shall include the following:
15.13.6.1.1 The length of leave requested and the dates and times within the fiscal year during which the leave will be taken.
15.13.6.1.2 The purpose for which the leave is requested.
15.13.6.1.3 The complete course of study to be pursued (if applicable). 15.13.6.1.4 The benefits to the District.
15.13.6.2 No more than three (3) percent of the Unit Members eligible to apply may be granted a Staff Development Leave each fiscal year. Staff Development Leaves may be limited to one (1) Unit Member within a department each fiscal year. In the event that a Board approved leave cannot be taken, alternate requests may be granted within the maximum percentage authorized each fiscal year.
15.13.6.3 A request for Staff Development Leave, accompanied by the Immediate Management Supervisor’s recommendation, shall be forwarded through established administrative channels. If the request is denied by the campus President/Xxxxxxx, or in the case of the District Administrative Offices, by the Chancellor or appropriate Chancellor’s Staff administrator, such decision shall be final. Criteria for recommending such leave shall be governed by:
15.13.6.3.1 The relative merit or reasons for desiring leave; 15.
Application Procedures. 1. Any teacher who has exhausted all other vested district sick leave days may make written application for benefits to the Screening Board. A written statement from the physician which outlines the teacher’s health status and inability to perform normal teaching duties must accompany the application for benefits.
2. The disposition of the request for benefits shall rest with the Sick Leave Screening Board. A written summary of the decision of the Sick Leave Screening Board shall be furnished to the applicant within three (3) calendar days after a decision has been made.
3. If benefits in excess of fifteen (15) days are utilized from the bank the teacher must submit an additional doctor’s statement. The Screening Committee may request the teacher to submit to a physical examination by a physician chosen by the Screening Board. The cost of said physical examination shall be borne by the school district.
Application Procedures. Employee makes application to Human Resources for funding to attend a course or seminar intended for career development.
Application Procedures. Completed and processed Free Application for Federal Student Aid (FAFSA) on file at Northwest State Community College (NSCC). Admitted to NSCC as a degree seeking student. Meet with your academic advisor or Xxxx to verify classes from Host Institution will be accepted and satisfy degree requirements at NSCC. Course being funded by Financial Aid must be required by your program of study. Have your departmental Xxxx sign Parts II of this agreement, this signature indicates that the classes will transfer to NSCC. Complete and sign the Student Information and Certification sections. Deliver this form to the Host Institution Financial Aid Office and Registrar’s Office for completion and signatures. Return this form to the NSCC Financial Aid Office when completed.
Application Procedures. (a) Applications must be received by 1 March with appropriate deductions commencing the following September.
(b) Applications shall specify the preferred financial arrangements and the school year in which the leave is requested. The year of leave shall commence no later than six (6) years after the date of the first deferral of salary.
(c) Applicants will be notified of acceptance or rejection by 1 May of the year of application.
(d) Subject to the Layoff and Recall provisions of this collective agreement the employee remains under employment with the Board during the year of absence, but is free to pursue whatever activity is decided on by the employee.
Application Procedures. Employees wishing to apply for posted vacancies will make application to the Human Resource office in the prescribed manner.
