Participant Contact Information. To direct communications to the Participant’s breach/security incident response staff, CDPH shall initiate contact as indicated by Participant in Attachment C. Participant’s contact information must be provided to CDPH prior to execution of this Agreement. Participant reserves the right to make changes to the contact information in Attachment C. Said changes shall not require an amendment to this Agreement.
