Patient Safety Card. ‌ A Patient Safety Card will be provided to patients to carry with them at all times. The card is provided to increase patient awareness of the risk of meningococcal infection and promote quick recognition and disclosure of any potential signs or symptoms of infection experienced during the course of the study and to inform patients on what actions must be taken if they are experiencing signs or symptoms of infection. At each visit throughout the study, the study staff will ensure that the patient has the Patient Safety Card and will review it with the patient at each visit as described in the SoA (Section 1.3).

  • Dimensions Education Bachelor’s Degree in Computer Science, Information Systems, or other related field. Or equivalent work experience. A minimum of 3 years of IT work experience in supporting desktop software and hardware products and problem solving/troubleshooting.

  • NOW, THEREFORE the parties hereto agree as follows:

Either party may terminate this DPA and any service agreement or contract if the other party breaches any terms of this DPA.

  • Severability Any provision of this Agreement that is prohibited or unenforceable in any jurisdiction shall, as to such jurisdiction, be ineffective to the extent of such prohibition or unenforceability without invalidating the remaining provisions hereof, and any such prohibition or unenforceability in any jurisdiction shall not invalidate or render unenforceable such provision in any other jurisdiction.
