Permitted Uses and Disclosures of the LDS. Recipient may use and/or disclose the LDS only for the Research described in this Agreement or as required by law.

Permitted Uses and Disclosures of the LDS. Recipient may use and/or disclose the LDS for the research purposes of the Pediatric Heart Transplant Society (PHTS) on behalf of the PHTS Foundation. In addition, Recipient may use and/or disclose the LDS for other research projects, as pre-approved by the Pediatric Heart Transplant Society Scientific Committee. Recipient will maintain a list of all research projects approved by the Pediatric Heart Transplant Society Scientific Committee.

a. Data used under this Agreement, except those with prior, independent REC approval, which are bound by the protocol approved, are not eligible for open source data sharing; this includes but is not limited to data that were provided anonymously and those for service evaluation, service improvement, and audit.
b. CCI reserves the right to ‘spot check’ any data usage without prior warning, in order to ensure proper usage.
c. Data use will be audited as part of CCI’s standard procedures and annual reporting.
Permitted Uses and Disclosures of the LDS a) Recipient may use and/or disclose the LDS for its Research and Public Health activities and the Health Care Operations of the Covered Entity, specifically including conducting a research Study entitled “[INSERT NAME OF STUDY]” (the “Study”). Recipient may use the LDS only in accordance with the purposes and procedures set forth in the Study protocol.
b) At the request of the Covered Entity, Recipient agrees to provide Covered entity a written summary of the procedures the Recipient uses to safeguard LDS.
