PHASE IA. The CONSULTANT shall physically re-inspect all bridges in accordance with time frames outlined in APPENDIX C (Schedule) and provide the LPA with one (1) hard copy of draft report and two (2) hard copies of the final report, accompanied by necessary maps, evaluations and recommendations. The CONSULTANT shall submit one (1) copy of the Final Report to ERMS, County Bridge Inspection Reports. The title page of each copy of the revised report shall include the signature, date and seal of the Professional Engineer in charge of the bridge inspection. Specifically, the following items shall be incorporated into the revised report which is in accordance with the requirements of PHASE I: Structure Inventory and Appraisal (SI&A) report for individual structures. Photographs of the bridge alignment and elevation. New photographs of areas showing deficiencies of any bridge exhibiting notable changes since the last report. Updated information in the INDOT Bridge Inspection Database shall be furnished to the Bridge Inspection Unit at INDOT. Updated Load ratings as necessary meeting the requirements of the INDOT Bridge Inspection Manual.
