Physician Statement For Use Of. Sick Leave An employee with an illness or disability exceeding two (2) days may be required to furnish a statement from his physician before returning to work, notifying the Employer that the employee was unable to perform his duties during the period of absence and is able to return to work. Where sick leave is requested to care for a member of the immediate family the Employer may require a physician’s certificate to the effect that the presence of the employee is necessary to care for the ill person.
