PHYSICIAN’S STATEMENT REQUIRED UPON RETURN FROM LEAVE. As a condition of reinstatement from a disability Leave of Absence taken under this Article, the employee must furnish a written statement from a physician verifying that he/she is physically able to perform his/her job functions. Securing this physician statement is at the employee's expense.
