Preventive Office Visits. This agreement provides coverage for the following preventive office visits. See Summary of Medical Benefits for the amount you pay. Adult Annual preventive visit - one (1) routine adult physical examination per plan year per member will be covered; Pediatric preventive office and clinic visits - Birth - 35 months: 11 visits; 36 months - 19 years: 1 per plan year; Well Woman annual preventive visit - one (1) routine gynecological examination per plan year per female member will be covered.
