Common use of Professional Services Consultant Clause in Contracts

Professional Services Consultant. The Form A or Form B Certifications and Disclosures submitted by the Level 1 Professional Services Consultant with the proposal documents are hereby made a part of this contract. The Supplemental University of Illinois Certifications identified in the Statutory Certifications section below in addition to the Form A or Form B Certifications and Disclosures are incorporated into this contract. It is the responsibility of the Level 1 Professional Services Consultant to provide the following with respect to each Level subconsultant* contracts which exceed $50,000. The documents submitted to the Owner shall be in electronic pdf format and follow the Owner’s file naming convention. The forms and file naming convention can be found at: xxxx://xxx.xxxxxxx.xxxxxxxxx.xxx/contractors/contracts. These documents shall be provided to the Owner within 15 calendar days after the execution of the Contract of after execution of the subcontract, whichever is later.
