PROPERTY DISCLOSURE FORM. Buyer ❑ has ❑ has not received the Ohio Residential Property Disclosure form 209 or ❑ Seller represents and warrants that Seller is exempt from providing the Ohio Residential Property Disclosure (Ohio 210 REALTORS® Residential Property Disclosure Exemption Form attached).
