Common use of Provider Notification to Members upon Termination Clause in Contracts

Provider Notification to Members upon Termination. Should PROVIDER terminate this Agreement pursuant to Section VII.1 above, or should PROVIDER move office location, or should a particular practitioner leave PROVIDER’s practice or otherwise become unavailable to the Member(s) under this Agreement, PROVIDER agrees to notify effected Member(s) a minimum of thirty (30) days prior to the effective date of such action or termination.
