Qualification Questions. Respondents shall submit a Yes/No response to the following Qualification Questions within MFMP Sourcing. A Respondent must meet the requirements identified and certify their compliance with the requirements through the following questions in order to be considered responsive and responsible. Respondents that answer “no” to any of the Qualification Questions shall be deemed non-responsive. 1. Does the Respondent certify that it is authorized to respond on behalf of its company to this RFP? 2. Does the Respondent certify that it is not on the Discriminatory or Convicted Vendors list as defined and identified in the PUR 1001? 3. Does the Respondent certify that it is not on the DMS Suspended Vendor List? 4. Does the Respondent certify that it is not on the Scrutinized Companies list as specified in Section 287.135 Florida Statutes? 5. Does the Respondent certify that its company has a current and active registration with the Florida Department of State, Division of Corporations, accessible at the following website: xxxx://xxxxxx.xxx/? 6. Does the Respondent agree: A. To provide delivery on a statewide basis within two business days for orders placed by 4:00 PM ET; and B. To provide F.O.B., the place of destination, freight prepaid, terms for the Desktop Location, or alternate location specified by the Customer; and C. To not impose delivery charges for orders above $25.00 for the life of the contract, including all contract renewals and extensions excluding furniture deliveries; and
