Qualified Medical Child Support Order (QMCSO). A medical child support order that: a. Either creates or recognizes the right of an alternate recipient (a Covered Person’s child who is recognized under the order as having a right to be enrolled under the Plan) or assigns to the alternate recipient the right to receive Benefits for which a Covered Person or other beneficiary is entitled under the group dental health plan; and b. Includes the name and last known address of the Covered Person and the name and address of each alternate recipient, a reasonable description of the type of coverage to be provided by the group dental health plan or the manner in which such coverage is to be determined, the period for which coverage must be provided, and each plan to which the order applies.
