Common use of Quarantine with documentation upon return Clause in Contracts

Quarantine with documentation upon return. Any employee within this contract shall be required to provide written evidence from a physician (MD or DO) or Chiropractic doctor as to the condition of the health of the employee after the third (3) consecutive day (not needed for days 1, 2 and 3) or at any time as requested by the employer for cause.
