Common use of Report of Incident or Injury Clause in Contracts

Report of Incident or Injury. When an employee is injured in the performance of work-related activities, the Employee shall immediately report the injury to their immediate supervisor and to the Workers’ Compensation Board using the forms supplied by the Employer in each Prairie South School Division location and on the PSSD website. (i.e. School Incident Report Form; Worker’s Compensation Board forms; Violent Incident Report Form as applicable).
