Common use of Reportable Events under Section III.I Clause in Contracts

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to OIG shall be made within 30 days of the identification of the Overpayment, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement a. a description of the steps taken by the Provider to identify and quantify the Overpayment; b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the Provider’s actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to OIG shall be made within 30 days of the identification of the Overpayment, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a description of the steps taken by the Provider Xxxxxxxxxx Regional to identify and quantify the Overpayment;; Xxxxxxxxxx Regional Medical Center Corporate Integrity Agreement b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the ProviderXxxxxxxxxx Regional’s actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to OIG shall be made within 30 days of the identification of the Overpayment, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a description of the steps taken by the Provider LWCI to identify and quantify the Overpayment; b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the ProviderLWCI’s actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to OIG shall be made within 30 days of after making the identification of the Overpaymentdetermination that a substantial Overpayment exists, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a description of the steps taken by the Provider Ocean Dental to identify and quantify the Overpayment; b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the Provider’s actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to OIG shall be made within 30 days of the identification of the Overpayment, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a copy of the notification and repayment to the payor required in Section III.H.2; b. a description of the steps taken by the Provider First Call to identify and quantify the Overpayment; b. c. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the Provider’s actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to OIG shall be made within 30 days of the identification of the Overpayment, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a description of the steps taken by the Provider Xxx Xxxxxx to identify and quantify the Overpayment; b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the Provider’s Xxx Xxxxxx’x actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.aIII.G.1.a, the report to OIG shall be made within 30 days of after making the identification of the Overpaymentdetermination that a substantial Overpayment exists, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a description of the steps taken by the Provider Providers to identify and quantify the Overpayment; b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the Provider’s actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to OIG shall be made within 30 days of the identification of the Overpayment, and shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a description of the steps taken by the Provider Medicus to identify and quantify the Overpayment; b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the ProviderMedicus’s actions taken to correct the Reportable Event; and

Reportable Events under Section III.I. a. For Reportable Events under Section III.I.1.a, the report to the OIG shall be made within 30 days of the identification of the Overpaymentoverpayment, and even if not yet quantified. The report shall include: Alliance/Active/Xxxxx/Xxxx/Xxxxxx Corporate Integrity Agreement: a. a description of the steps taken by the Provider Xxxxxxxx to identify and quantify the Overpayment; b. a complete description of the Reportable Event, including the relevant facts, persons involved, and legal and Federal health care program authorities implicated; c. a description of the ProviderOrthofix’s actions taken to correct the Reportable Event; and
