REPORTING AND EVALUATION REQUIREMENTS. 7.1. Unless another reporting schedule has been required or approved by Grantor, Grantee agrees to submit the following minimum data to Grantor on a quarterly basis, with quarters beginning at the start of the calendar year, within 15 days following the quarter covered by the report: (a) Victim Statistics: Total number of victims and significant others served by program, type of crime, type of services provided, race, sex, age, national origin and disability, where such information is voluntarily furnished by those receiving services; and
