Final Evaluation. IC must submit a final report and a project evaluation to the Arts Commission within thirty (30) days after the completion of the Services. Any and all unexpended funds from IC must be returned to City no later than sixty (60) days after the completion of the Services.

Final Evaluation. 12.5.3.1 A written evaluation shall be provided to the unit member no later than thirty (30) days before the last scheduled school day for the school year in which the evaluation takes place. 12.5.3.2 The written evaluation shall be placed in the unit member’s personnel file. 12.5.3.3 The unit member may write a response to the evaluation and it shall be an attachment to the written evaluation. Such response shall be submitted within 30 days of receipt of the evaluation. 12.5.3.4 No unit member shall be requested to backdate a written evaluation.
Final Evaluation. The M&E Plan will make provision for final Project level evaluations (“Final Evaluations”). With the prior written approval of MCC, MCA-Philippines will engage independent evaluators to conduct the Final Evaluations at the end of each Project. The Final Evaluations will review progress during Compact implementation and provide a qualitative context for interpreting monitoring data and impact evaluation findings. They must at a minimum (i) evaluate the efficiency and effectiveness of the Activities, (ii) determine if and analyze the reasons why the Compact Goal, Program Objective and Project Objective(s), outcome(s) and output(s) were or were not achieved, (iii) identify positive and negative unintended results of the Program, (iv) provide lessons learned that may be applied to similar projects, and (v) assess the likelihood that results will be sustained over time.
Final Evaluation. At the conclusion of the evaluation period, the evaluatee shall present to the evaluator the results of the agreed-upon evaluation plan. The evaluator shall review the results and complete a summary evaluation form for inclusion in the evaluatee's file.
Final Evaluation. 14.6.4.1. The evaluator and the evaluatee complete and sign Part D of the evaluation worksheet and performance evaluation addendum (if any). The evaluator prepares a draft copy of the summary evaluation report for discussion with the evaluatee according to the following schedule/ procedure: a. On or before the third Friday in February for second year probationary unit members. b. On or before the last workday in April for permanent unit members, first year probationary unit members, and leave replacements. This timeline may be extended up to May 15 for unit members assigned to year-round sites. 14.6.4.2. Not later than thirty (30) calendar days prior to the last school day of the school calendar, the evaluatee shall sign and be provided with a copy of his/her evaluation. The evaluatee may, within thirty (30) calendar days, attach a written response to the evaluation which shall become a permanent part of the personnel file. 14.6.4.3. In the event an evaluatee has been evaluated as "Requires Improvement" or "Unsatisfactory" in any area of his/her evaluation, the evaluator may continue the evaluation process for the balance of the current school year. The evaluator may attach an addendum to the summary evaluation indicating the results of the continuing evaluation, as it relates to the areas identified as "Requires Improvement" or "Unsatisfactory." Should the performance fail to improve to “satisfactory”, the evaluatee will be scheduled for a special evaluation during the next school year. If the "Requires Improvement" or "Unsatisfactory" ratings are changed to "Satisfactory," a summary evaluation which reflects the improved performance will be prepared and signed by both Parties. The evaluatee shall sign the addendum and may, within thirty (30) calendar days, attach a written response to the addendum which shall become a permanent part of the personnel file.
Final Evaluation. At the end of the 14 calendar day draft period, and after considering the employee’s comments and suggestions, the supervisor will issue the final evaluation, incorporating any changes he/she feels are warranted.
Final Evaluation. Evaluation and assessment made pursuant to this procedure shall be reduced to writing and a copy thereof shall be transmitted to the employee not later than thirty (30) days before the last school day of the school year in which the evaluation takes place. A conference between the evaluator and the employee to discuss the evaluation shall be scheduled. The employee shall have the right to initiate a written reaction or response to the evaluation. Such response shall be attached to the employee's evaluation. Said employee shall not be evaluated again within the same school year. Final Evaluation. Within fifteen (15) calendar days after completion of the probation period, a SER shall be completed by the evaluator and discussed with the probationary staff member. The staff member is able to request the presence of an Association representative in the conference. The inability of the Association representative to meet within the fifteen (15) days will not lead to a grievance. The staff member must be removed from probation if he or she has demonstrated improvement in those areas specifically detailed in his or her initial notice of deficiency and subsequently detailed in his or her program for improvement to the satisfaction of the evaluator, which ultimately results in a new SER of level 3 or above.

Final Evaluation. 5.1 A final overall evaluation rating for a permanent classroom teacher shall only be made when the unit member is found to be "Unsatisfactory." If no unsatisfactory rating is made, the teacher is determined to have satisfactorily completed the evaluation process. A final overall evaluation for unit members other than classroom teachers shall be determined as “satisfactory” or “unsatisfactory.” 5.2 A classroom teacher in probationary status will not be given an overall rating on his/her Final Evaluation. Key Elements on the Probationary Teacher Performance Evaluation Form will only be marked “Met” or “Needs Improvement.” A classroom teacher in temporary status who is placed on formal conference evaluation may receive an overall rating of “Unsatisfactory.” 5.3 A standard is determined to have been met if a majority of the Key Elements which support a specific standard have been marked "Met" on a classroom teacher's Formal Performance Evaluation Form. 5.3.1 The Rubric will be used to determine whether or not a Key Element has been met. The Rubric is included as an exhibit in this Agreement. 5.3.1.1 Since the Key Element Rubric performance indicators may be listed more than once in support of a Key Element, consideration will be taken when using such indicators to support evidence of failure to meet a Key Element or a standard. 5.4 In preparing the Final Evaluation the evaluator shall rely upon data collected through observations, collection of relevant data as described in Section 3.1, and evaluation conferences with the evaluatee. 5.5 An overall Unsatisfactory rating will be determined when the permanent classroom teacher fails to meet at least two of the three designated standards. (Or a standard subsequently substituted by the end of the first quarter). 5.5.1 An "Unsatisfactory" rating on the final formal evaluation form of a permanent classroom teacher will be accompanied by related and specifically documented evidence on each Standard, which is determined to be "Not Met." 5.6 In circumstances where, in the principal’s professional judgment, student learning is in serious jeopardy or where the work of others is negatively impacted to a serious degree, a determination of "Unsatisfactory" for a permanent classroom teacher may be made if only one standard is designated as "Not Met." 5.7 A rating of Unsatisfactory on the Final Performance Evaluation Form for a permanent classroom teacher shall result in a referral to the Partnership Program. 5.8 A copy of th...
