Common use of REPORTING AND EVALUATION REQUIREMENTS Clause in Contracts

REPORTING AND EVALUATION REQUIREMENTS. 8.1. CONTRACTOR shall complete all reporting and evaluation activities as required by the COUNTY and described herein. 8.1.1. CONTRACTOR shall provide reports as requested by COUNTY including but not limited elements identified in the Practice Guidelines, Quality Assurance Work Plan/Quality Improvement Work Plan, Compliance Plan, Cultural Competence Plan, or any standard measures required by DHCS and any data specified that enables COUNTY to measure the CONTRACTOR’S performance. CONTRACTOR shall use templates as determined by COUNTY when requested. 8.1.2. CONTRACTOR shall submit annual quality improvement plans and quarterly outcome reports if requested. 8.1.3. CONTRACTOR performance measurements shall include child or youth progress data, child or youth and parent/guardian satisfaction with services, intakes, discharges, and other relevant outcome data CONTRACTOR shall collect and report to COUNTY. 8.1.4. CONTRACTOR shall track the number and type of complaints received during the year by its agency for review by its staff, and COUNTY CSOC leadership and Quality Improvement staff. 8.1.5. Quarterly Reporting/Audit Requirements: The CONTRACTOR shall submit quarterly reports indicating open cases, as well as services provided. The report shall also include a synopsis of all cases that closed in the prior quarter, as well as the level of client and family functioning upon closure. The CONTRACTOR shall make charts available for Placer County and/or State and Federal auditors to audit for Medi-Cal regulation compliance, upon notice. Notice may be as little as 24 hours in the case of a State or Federal audit, including EPSDT and DHCS audits. The CONTRACTOR will also perform internal monthly chart audit activities with a report of such activities sent to the County with the quarterly report of open cases and services provided.
