Reporting of Unauthorized Uses or Disclosures and Security Incidents. Business Associate agrees to report to Covered Entity in writing any access, use or disclosure of the PHI not provided for or permitted by this Agreement and, any Security Incidents of which Business Associate (or Business Associate’s employee, officer or agent) becomes aware. Business Associate shall so notify Covered Entity pursuant to this Section within twenty-four (24) hours after Business Associate becomes aware of such unauthorized use, disclosure or Security Incident.

Reporting of Unauthorized Uses or Disclosures and Security Incidents. The Service Arrangement includes obligations to notify Covered Entity of a Security Breach as that term is defined in the Service Arrangement. Business Associate will report to Covered Entity any Use or Disclosure of PHI not provided for by the Service Arrangement of which Business Associate becomes aware, and Security Incidents of which Business Associate becomes aware, provided that notice is hereby deemed given for Unsuccessful Security Incidents.
