Reporting Requirements and Schedule. The Borrower, or its Contractor and Subcontractors, must submit the following reports to the City’s Department of Civil Rights (DCR) within the timeframes and methods outlined below. a) Submit WHEDA’s Emerging Business Enterprise (EBE) Participation Plan before construction begins to xxxxxxxxxxxxxxxxxx@xxxxxxxxxxxxx.xxx. b) Submit a list of all subcontractors before construction begins using DCR’s standard form to the email address above: xxxxx://xxx.xxxxxxxxxxxxx.xxx/civil-rights/documents/SubcontractorList for Nonpublic Works.xlsx. DCR must be notified of subcontractors added after the start of construction by submitting an updated subcontractor list within 30 days of subcontractor award. c) The City’s Committed Cost Status Report (CCSR) and the City’s Monthly Employee Utilization Report (MEUR) must be entered online by the 15th of each month through Citizen Access during the construction or rehab period. The first reports are due the month after construction begins. In addition, the Contractor shall describe the efforts it undertook to prevent use or submission of covered telecommunications equipment or services, and any additional efforts that will be incorporated to prevent future use or submission of covered telecommunications equipment or services.

  • Subsidy Requests and Reporting Requirements 1. The Grantee or Management Company shall complete a CRF Subsidy Request Report - Recap of Tenant Income Certification, which provides a unit-by-unit listing of all units in the Development for whom assistance is being requested and gives detailed information including the occupants’ eligibility, set-aside requirements, amount of household rent paid, utility allowance and amount of CRF Rental Subsidy requested. 2. The CRF Subsidy Request Report - Recap of Tenant Income Certification shall be prepared as of the last day of each calendar month during the period of performance and shall be submitted to XXXXxxxxxxxx@XxxxxxxXxxxxxx.xxx and Florida Housing’s monitoring agent no later than the 15th day of the following month. The December 2020 request will be due on or before December 15th. The Grantee will submit executed Coronavirus Relief Fund Rental Assistance Applications and supporting documentation to Florida Housing’s monitoring agent within 5 days upon the monitoring agent’s request.
