Reporting Forms. A. Annual Growth Plan - to be completed annually for each staff member whose performance is considered satisfactory. This form is to be a cooperative effort by the individual staff member and the responsible administrator. Where appropriate, a supervisor's input will be included. This form must be signed and returned to the Superintendent's Office by March 1 of each school year. A copy of all revisions will be forwarded to the Superintendent. B. Performance Improvement Plan - to be completed for each staff member whose performance is considered unsatisfactory. It is understood that progress toward this plan, together with other pertinent information, will be used to make decisions regarding employment status. This form must be signed by the appropriate parties and returned to the Superintendent's Office by March 1. With the permission of the staff member, a copy of the Performance Improvement Plan will be forwarded to the President of the Concord Education Association. C. Annual Recommendation Form - to be completed by administrators for each staff person assigned to their building or program. This from should be accompanied by either an Annual Growth Plan, or a Performance Improvement Plan. This form should be returned to the Superintendent's Office by March 1 of each school year. D. Guidelines For Certified Staff - to be used by staff members, administrators, and supervisors as a guide when completing the Annual Growth Plan or the Performance Improvement Plan. These documents are not to be considered as checklists or to be included with other evaluation documents. E. Three-Year Plan - By state law and school district policy, staff members are required to develop with their administrators a Three-Year Plan of professional activities at the start of each recertification cycle. For details see Staff Development Master Plan.
Reporting Forms. The Employer shall report to the Administrative Manager of the Plans for all covered employees for the month on forms provided by the Trustees of the Plans. It shall be the obligation of the Employer to have and use the official reporting forms. If the Employer maintains his payroll records and information on computer or other electronic equipment and desires to use and submit the required information in the form printed out by the computer or other electronic equipment, the Employer may use and submit such forms other than official reporting forms; provided, however, the Trustees shall have the right to reject such forms if they are not consistent or reconcilable with the official form. The reports shall be on a calendar month basis, unless otherwise agreed upon in advance by the Trustees and the Employer, subject to the provision below.
Reporting Forms. The CONTRACTOR shall furnish the STATE with any additional reports or data that may be required by FTA or other federal agencies. Such reports and/or data will be submitted on forms provided by the STATE.

XXXXXX sends the amended copy of the test repot back to the SUPPLIER. The procedure is such that the signed copies remain with the SUPPLIER and XXXXXX.

Reporting Forms a) All Participating Employers shall report to the Administrator(s) of the aforesaid Employee benefit plans, or such other duly appointed depository, for all hours paid (or otherwise contributed for) by all Employees participating in the Employee benefit plans on forms provided by the Trustees of the Plans. It shall be the obligation of the Employers to have and use the official reporting forms. If an Employer maintains his payroll records and information on computer or other electronic equipment and desires to use and submit the required information in the form printed out by the computer or other electronic equipment, the Employer may use and submit such forms other than official reporting forms. The Administrator, however, reserves the right to reject such forms. b) All reports shall be for the full calendar month last preceding. However, the Employer may use other reporting periods subject to rejection by the Administrator.
