Reporting Serious Incidents. 7.1 Contractor shall report serious incidents to the BHRS Director or the Director’s designee (i.e., BHRS QM Unit). A serious incident is any event which jeopardizes the health and/or safety of clients, staff and/or members of the community including, but not limited to, physical injury and death. Serious Incidents are categorized into two levels. 7.1.1 Level One are high-risk events, such as incidents reported in the media, death or serious physical injury on the Program premises, or any incident that is a significant deviation from the usual process of providing behavioral health services. 7.1.2 Level Two incidents are all other types of serious incidents. 7.2 All serious incidents shall be reported to BHRS QM Unit. Level One serious incidents shall be reported immediately upon knowledge of the incident. Level Two serious incidents shall be reported within three (3) calendar days of the event or as soon as possible after becoming aware of the serious incident. Reports shall include the following elements: 7.2.1 Complete written description of event including outcome; 7.2.2 Written report of Contractor’s investigation and conclusions; 7.2.3 List of persons directly involved and/or with direct knowledge of event. 7.3 County and the State of California, Department of Health Care Services, retain the right to independently investigate serious incidents and Contractor will cooperate in the conduct of such independent investigations.
