Reporting to Employer. Employees must report immediately all work related injuries or illness and non-work related injury or illness that may affect his or her ability to carry out his or her duties or regularly attend work. Employees and a Health Care Practitioner must complete and submit the required forms as soon as practicable. The Employer shall provide the employee with a copy of the completed WSIB Form 7 upon submission.
