Request for Participation. 1. The lead organization should submit a request for participation to other NRC offices (e.g., FSME, the Office of Nuclear Material Safety and Safeguards (NMSS), the Office of the General Counsel (OGC), the regions). When the lead organization is the Organization of Agreement States (OAS), requests for NRC office participation should be made through FSME.
2. If Agreement State representation is desired, the lead organization will contact FSME to discuss Agreement State involvement. The lead organization should provide the purpose, scope of activities, expected product or outcome of the working group, and the estimated level of effort expected of participants to FSME for its use in seeking Agreement State participation. Exhibit 2 is a sample request for participation. Each area is discussed further below.
3. The request should identify the working group as a FSME working group as defined in this section and include—
Request for Participation. I wish that my child participate in activities conducted by ▇▇▇▇▇▇ Gospel Church, Dallas, TX, (hereafter termed “the Church”). This release covers field trips, sports, instruction, guidance, discipline, and all other activities of these programs in which my child participates. My child may receive pastoral counseling if my child indicates a need to speak confidentially with a minister. I give permission for Church ministers to provide individual spiritual guidance to my child as in their best discretion they deem appropriate. I desire that this release shall continue in effect until a new release is signed by me, or until I revoke in writing this release. I understand that no revocation will be effective unless I notify the Church in writing.
Request for Participation. No request for participation in MIPIM 2018 shall be deemed to have been accepted by us unless and until we receive your signed agreement to these Terms and Conditions and we have confirmed in writing that your request has been successful (“Confirmation”). We reserve the right to reject any request for participation.
Request for Participation. I hereby apply for participation in the group insurance trust policy identified above. To the best of my knowledge, all responses on this application are complete and true. I understand and agree that:

Each Employee who is eligible to participate in this Agreement and properly executes an Employee Application which is accepted by the Custodian thereby becomes a party to this Agreement with the right to enforce its terms against any other party. Such executed Employee Application is hereby specifically incorporated herein by reference. An Employee Application is properly executed when completed and signed by the Employee. The Custodian may rely on the validity of the signature thereon, on the existence of the employment relation thereby affirmed, and on the irrevocable subscription to the provisions of this Agreement therein contained.
Request for Participation. Each Employee who properly executes an Application thereby becomes a party to this Agreement with the right to enforce its terms against any other party. Such executed Application is hereby specifically incorporated herein by reference. An Application is properly executed when signed by the Employer, the Employee and the Custodian. The Custodian may rely on the validity of the signatures thereon, on the existence of the employment relation thereby affirmed, and on the irrevocable subscription to the provisions of this Agreement therein contained.
