Requesting Agency Notice For Request. To Work 1) Requesting Agency shall, when possible, provide four (4) calendar days (96 hours) advance notice to the Contractor when requesting Reporter or LVS services. There may be occasions when Requesting Agency requires a Reporter or LVS within three (3) calendar days (72 hours) advance notice, or less in special circumstances. The Contractor shall make every effort to supply a Reporter for services requested under these circumstances. 2) Requesting Agency shall inform Contractor that services are being requested through use of this MSA. Requesting Agency should identify Contractor’s assigned MSA contract number when contacting Contractor. 3) Requesting Agency should provide Contractor the following information when ordering services: a) Type of services required – indicate specific Service Category (1 thru 6) include any required “Add-on” services b) Date of the hearing or deposition c) Time d) Location e) A caption - case name f) Name of hearing officer, judge, attorneys, or witnesses/speakers (as applicable) g) Number of participants (if known) h) Public notice, unusual terms, any written material received from speakers (as applicable). i) Anticipated duration of hearing or deposition j) Whether or not a transcript is required (if known) along with anticipated transcript turnaround time k) Number of Days (after hearing/deposition) to deliver final transcripts l) Advance notice of production of a final transcript by 7:00 P.M. on the same day as the proceeding

  • Request for Review Within sixty (60) days after receiving notice from the Plan Administrator that a claim has been denied (in part or all of the claim), then claimant (or their duly authorized representative) may file with the Plan Administrator, a written request for a review of the denial of the claim.   • Procedures for LNP Request The Parties shall provide for the requesting of End Office LNP capability on a reciprocal basis through a written request. The Parties acknowledge that Verizon has deployed LNP throughout its network in compliance with FCC 96-286 and other applicable FCC Regulations. 15.4.1 If Party B desires to have LNP capability deployed in an End Office of Party A, which is not currently capable, Party B shall issue a LNP request to Party A. Party A will respond to the Party B, within ten (10) days of receipt of the request, with a date for which LNP will be available in the requested End Office.   • Notice of Registration If at any time or from time to time the Company shall determine to register any of its securities, either for its own account or the account of a security holder or holders, other than (i) a registration relating solely to employee benefit plans, or (ii) a registration relating solely to a Commission Rule 145 transaction, the Company will: (i) promptly give to each Holder written notice thereof; and (ii) include in such registration (and any related qualification under blue sky laws or other compliance), and in any underwriting involved therein, all the Registrable Securities specified in a written request or requests, made within twenty (20) days after receipt of such written notice from the Company, by any Holder.
