AGENCY SHALL. A. Provide facilities as presently available and as necessary for the development and maintenance of a clinical/educational experience for students of the program. B. Maintain the Agency facilities used for the clinical/educational experience in such a manner that said facilities shall conform to all requirements of applicable State Boards, and/or Business and Professional Codes. C. Assure that staff is adequate in number and quality to insure safe and continuous management of the student program in cooperation with the Institution’s instructor. D. Provide Instructors and students taking part in the field experience, whenever possible, other incidentals that may be mutually agreeable. E. Provide emergency first aid for any student who becomes sick or injured by conditions arising out of or in the course of said student’s participation in the clinical experience at the Agency. Provide medical examinations or other protective measures that may be required by the Agency. F. Have the right, after consultation with the Institution, to refuse to accept for further clinical/educational experience any of the Institution’s students who in the Agency’s judgment, are not participating satisfactorily.
AGENCY SHALL. 17 1. Review service levels contained within COUNTY’s preliminary annual Work Plan, and 18 make adjustments by October 1, based upon AGENCY’s budget and desired service levels and 19 consistent with available COUNTY resources and contract capability. 20 2. Meet periodically or as needed with COUNTY to discuss the services being provided. 21 Review with COUNTY any cost overruns associated with deviations from the Work Plan at AGENCY 22 request and develop strategies for budget and/or service levels commensurate with available funding. 23 3. Notify COUNTY of priority service requests to be included within the following two-week 24 work schedule. Review work schedule for force account and contract work. 1 4. Designate an individual or individuals authorized to request performance of services 2 hereunder and designate an individual or individuals with authority to review and approve contract 3 documents on behalf of the AGENCY. 4 5. Subject to the limitations specified herein, pay COUNTY for entire total cost incurred by 5 COUNTY for maintenance, operation, support and contract functions performed within the SJH 6 Maintenance Areas by COUNTY staff or COUNTY administered contract to include all costs for labor, 7 equipment, materials, service contracts, special maintenance contracts, repair contracts, and all 8 applicable overhead and supervisory expenses in accordance with established COUNTY prices, rates, 9 fees and charges at the time work is performed. 10 6. Pay COUNTY invoiced costs by payment due date. Payment due dates are date of 11 delivery of acceptable invoice plus thirty days. 12 7. Pay late charges for invoices not paid within thirty (30) days after receipt of in accordance 13 with the following schedule: 14 a. Over-the-counter payments will be assessed a late charge if any payment is not 15 received by the COUNTY by the payment due date. 16 b. Payments transmitted to the COUNTY via the U.S. Mail that have payer’s postage 18 payment due date plus one day. 19 c. Payments transmitted to the COUNTY via the U.S. Mail that have a U.S. Post 20 Office postmark dated after the payment due date will be assessed a late charge. 21 d. AGENCY SHALL. 1. Submit all applicable reports to Kitsap County by the published deadlines and respond in a timely way to County information requests and countywide reports. 2. AGENCY SHALL a. Acquaint School’s Liaisons with the AGENCY’s overall objectives of the Field Experience for Students and provide all appropriate educational objectives and documents for the Field Experience to School;
AGENCY SHALL. Permit each student who is designated by the University to receive a social work field practicum at the Agency and shall furnish, and permit such students and designated field instructors free access to appropriate Agency facilities for the field practicum.

AGENCY SHALL. I. At its sole expense without contribution from State, maintain the Property, the Tower, and Shelter, and access thereto in good and tenantable condition, subject to reasonable wear and tear and damage from the elements. Notwithstanding the foregoing, if the Tower or Shelter is damaged to a weather event or other out of the ordinary event, State and Agency shall work cooperatively to seek a solution to meet the needs of both Parties. II. Agency shall be responsible for paying all utility charges for electricity service. Agency shall not be held responsible for utility service problems that are not willfully caused by Agency. If power for the Tower and Shelter is sub-metered from the other buildings Agency has on the Property, Agency may invoice State for its proportionate share of the power cost calculated on a percentage based on the number of racks State is using divided by the total number of racks in use at the site. III. Maintain its equipment properly licensed by the FCC and shall provide a copy of the FCC license to the State upon request. IV. Maintain its own equipment at the Site in good condition, reasonable wear and tear and damage from the elements excepted.
AGENCY SHALL a. Possess and maintain, at Agency’s expense, any license required by law to perform services under this Agreement. b. Notify Insurer in writing of any expiration, termination, revocation, suspension or any other action by a Department of Insurance or any other governmental agency affecting licenses required to perform services under this Agreement within five (5) business days of such occurrence.

AGREEMENT BETWEEN THE CALIFORNIA STATE UNIVERSITY, CALIFORNIA STATE
AGENCY SHALL. A. Permit each student who is designated by the University pursuant to Paragraph II.A below to receive clinical experience at the Agency and shall furnish and permit such students and University instructor’s reasonable access to appropriate clinical facilities for such clinical experience. B. Furnish appropriate clinical facilities, on a rotational basis, in such a manner that there will be no conflict in the use thereof between the University's student and students from other educational institutions, if any. C. Maintain the clinical facilities used for clinical experience in such a manner that said facilities shall at all times conform to the requirements of the curriculum requirements of the University. D. Assure the staff is adequate in number and quality to insure safe and continuous health care to individuals. E. Provide registered, licensed or certified clinical/professionals to supervise students when performing services for a patient. Students can only perform services when under their supervision. Such registered professionals are to be certified or licensed in the discipline in which supervision is provided. Students shall work, perform assignments, and participate in ▇▇▇▇ rounds, clinics, staff meetings, and in-service educational programs at the discretion of their supervisors designated by the Agency. Students are trainees, not employees, and are not to replace Agency staff. F. Provide first aid and other emergency treatment on-site, including, but not limited to, immediate evaluation for risk of infection and appropriate follow-up care of student in the event of a needle stick injury to or other exposure of student to blood or body fluids. In the case of suspected or confirmed exposure to the human immuno-deficiency virus (HIV), such follow-up care shall include, but not be limited to, the option for appropriate post-exposure HIV testing and the option for prophylactic therapy with zidovudine (AZT) and/or other appropriate pharmacologic agents as medically indicated. G. Permit and encourage members of the Agency and/or resident staff of the Agency to participate in the instructional phase of the clinical experience. H. Permit the Agency Program Supervisor and other designated personnel to attend meetings of the University's faculty or any committee thereof, to coordinate the clinical experience program provided under this Agreement.
