Required Notifications. 1. To be eligible for COBRA, the sub- xxxxxxx or applicable member must notify MCHCP of a divorce, legal separation, a child turning age twenty-six (26), or Medi- care entitlement within sixty (60) days of the event date. 2. The human resource/payroll office of the subscriber must notify MCHCP of an employee’s death, termination, or reduction of hours of employment. 3. If a COBRA participant is disabled within the first sixty (60) days of COBRA coverage and the disability continues for the rest of the initial eighteen- (18-) month period of continuing coverage, the member must notify MCHCP that s/he wants to continue coverage within sixty (60) days, starting from the latest of: 1) the date on which the SSA issues the disability determination; 2) the date on which the qualifying event occurs; or
