Common use of Right of Access to Information Clause in Contracts

Right of Access to Information. Business Associate agrees to provide access, at the request of Covered Entity, to protected health information in a Designated Record Set, either to the Covered Entity, or as directed by Covered Entity to an Individual. This right of access shall conform with and meet the requirements of 45 CFR 164.524, including substitution of the words “Covered Entity” with Business Associates where appropriate.

Right of Access to Information. Business Associate agrees to will provide access, at the request of Covered EntityPractice, to protected health information Protected Health Information in a Designated Record SetSet (as that term is defined in 45 CFR 164.501 of the Privacy Rule), either to the Covered EntityPractice or, or as directed by Covered Entity Practice, to an Individual. Right of Access to Information. Subject to the terms of this Agreement, the Business Associate agrees to provide accessaccess to Protected Health Information in a Designated Record Set (if applicable) to the Covered Entity or, at if directed by the request of Covered Entity, to protected health information an Individual in a Designated Record Setorder to meet the requirements under 45 C.F.R. 164.524, either to at the written request of the Covered Entity, or as directed by Covered Entity to an Individual. This right of access shall conform with and meet the requirements of 45 CFR 164.524, including substitution of the words “Covered Entity” with Business Associates where appropriate.
