Severity Assessment Clause Samples
The Severity Assessment clause defines the process for evaluating the seriousness or impact of a particular event, issue, or breach within the context of the agreement. Typically, this involves categorizing incidents based on predefined criteria such as risk level, potential harm, or disruption to services, and may require the parties to follow specific procedures for assessment and reporting. By establishing a clear framework for determining severity, this clause ensures that appropriate responses are triggered and resources are allocated efficiently, ultimately helping to manage risk and minimize negative consequences.
Severity Assessment. 3.1 Each force PSD will nominate a single point of contact (SPOC). The SPOCs will under take a severity assessment of the complaint or incident.
Severity Assessment. The investigator will assess the severity of all adverse events using the ***, ***, or *** to describe the maximum intensity of each adverse event. For purposes of consistency, these intensity grades are defined as follows: · ***; · ***; · ***. Note the distinction between the severity and the seriousness of an adverse event. A *** is not necessarily a ***. For example, a headache may be *** but would not be classified as serious unless it met one of the criteria for ***.
Severity Assessment. Investigators will assess the severity of all adverse events using the adjectives MILD, MODERATE, or SEVERE to describe the maximum intensity of each adverse event. For purposes of consistency, these intensity grades are defined as follows: • MILD: Does not interfere with the subject’s usual function; • MODERATE: Interferes to some extent with the subject’s usual function; or • SEVERE: Interferes significantly with the subject’s usual function. Note the distinction between the severity and the seriousness of an adverse event. A severe event is not necessarily a serious event. For example, a headache may be severe (interferes significantly with subject’s daily function) but would not be classified as serious unless it met one of the criteria for SAEs listed in Section 11.1.5.
