Resident declares that he/she is not currently required to register under the provisions of the Oklahoma Sex Offenders Registration Act or the Xxxx Xxxxx Violent Crime Offenders Registration Act. In the event Resident should be required to register under the provisions of the Oklahoma Sex Offenders Registration Act or the Xxxx Xxxxx Violent Crime Offenders Registration Act, he/she must notify the INTEGRIS Graduate Medical Education Office immediately and shall not be allowed to participate in any clinical activities at INTEGRIS Health, Inc.
