Signed Confidentiality Acknowledgement Form. Vendor will have Vendor Staff execute Exhibit B, Confidentiality Acknowledgement Form, which is due within thirty (30) calendar days of the execution of this Agreement and annually within twelve (12) months from the most recent date of approval in CAIS.
