Statutory Declaration to be given with Application. The application for long paternity leave must be accompanied by a statutory declaration made by the Employee stating the following
(a) particulars of any period of maternity leave sought or taken by his spouse;
(b) that the Employee intends to be the child’s primary care giver at all times while on long paternity leave; and
(c) that the Employee will not engage in any conduct inconsistent with his contract of employment while on long paternity leave. The Employee will not be in breach of clause 15.5.2 if he could not comply because of circumstances beyond his control.
