Type “Privacy & Confidentiality” in the search box and select “Go” Click ‘Select this Course’ Click “Take Course Now” All Study Team members will sign Grand River Hospital’s Confidentiality Agreement. The Principal Investigator shall provide a Data Collection Sheet (Appendix C of this Agreement). If data elements change, an amendment to this Agreement must be made at least 7 business days prior to extraction. The Principal Investigator shall provide an amended Data Collection Sheet for each change.
