Common use of Submission of Form Clause in Contracts

Submission of Form. A declaration or medical certificate must be submitted by the employee to the Employing Authority within ten (10) calendar days of the beginning of the absence. When an employee fails to furnish such a declaration or certificate within the required time, the employee shall not be paid for the period of absence unless there are extenuating circumstances to be decided by the Employing Authority.
