This is: • 68% of the GMS and GP Visit Card population. • 30% of the total population over 18 (GMS/GP Visit Card Population as of March 2018, Census 2016). A net target figure of 1,397,187 takes account that the Chronic Care Programme is a surgery based programme of care which will be delivered in GP centre(s) of practice only.
