Termination of Agreement by CareFirst or the Exchange. CareFirst or the Exchange may terminate the Agreement or the enrollment of a Subscriber and/or a Dependent under the following circumstances by providing notice of termination, including the termination date and the reason for termination, to the Subscriber or Application Filer promptly and without undue delay.

CareFirst or the Exchange may terminate the Agreement or the enrollment of a Subscriber and/or a Dependent at any time by written notice delivered or mailed to the last address as shown by the records of CareFirst or the SHOP Exchange under the following circumstances by providing at least five (5) days’ notice of termination, including the termination date and the reason for termination, to the Subscriber or Application Filer promptly and without undue delay.
