Termination of Patient Relationship. If Provider chooses to terminate a provider- patient relationship with a Member, either for cause or due to a change in the Provider’s availability, each Member must be given at least 30 days written notice in order to select another provider. During the 30 day period following receipt of the notice by the member, the Provider shall remain responsible for Emergency care for the Member. A copy of the notice terminating the provider-patient relationship should be sent to Crescent and to Payor or TPA.
