Common use of Termination Report (TR) Clause in Contracts

Termination Report (TR). 5 Within fifteen (15) days of termination of services for 6 each FAMILY/CLIENT terminating services during the preceding month. This TR 7 shall include, but not be limited to: 8 8.1.3.1 A summary of all information required on the 9 Monthly Progress Report identified in Subparagraph 8.1.2 of this Exhibit A, 10 8.1.3.2 All community resources/referrals given by 11 CONTRACTOR to CLIENT for follow-up services, 12 8.1.3.3 CLIENT’s ongoing issues/concerns, 13 8.1.3.4 The reason services were terminated, and 14 8.1.3.5 Other items pertinent to this Exhibit A.
