THE INQUIRY REPORT. The inquiry report shall contain the following information: (A) The name and position of the respondent(s); (B) A description of the allegations of research misconduct; (C) The federal or sponsor support involved, including, for example, grant numbers, grant applications, contracts, and publications listing support; (D) The basis for recommending that the alleged actions warrant an investigation; and (E) Any comments on the report by the respondent or the complainant.
