TO BE ELIGIBLE FOR IPP. (i) a new Employee shall commence coverage under the plan on the first working day following completion of their probationary period. (ii) an Employee who is not at work on becoming eligible will commence coverage following his/her return to work (iii) in order to be eligible for payment of short term disability benefits, the employee must submit a sick claim form, attached Appendix “B” for an absence that is extending to the sixth (6th) working day.
