User Participation. 5.1 Some of your participation in The Exchange may occur via online forums, blogs, chats or focus groups which SBS or its agentwill set up. While SBS encourages robust debate and discussion, all users should treat each other with tolerance, courtesy and respect.
5.2 You agree not to use any obscene, indecent, or offensive language or to place on the Website any material that is defamatory, abusive, harassing, racist, or hateful. Further, you may not place on the Website any material that is encrypted, constitutes junk mail or unauthorised advertising, commercial offers, invades anyone's privacy, or encourages conduct that would constitute a criminal offence.
5.3 You agree to use the Website only for lawful purposes. You may be held legally responsible for the material you submit to The Exchange.
5.4 All contributions to the Website are published at SBS’s sole discretion. Your contribution may be edited or removed from the Website, including if SBS considers it to be:
(a) false, misleading or in breach of any law, whether the laws of Australia or another jurisdiction, and including defamation, contempt of court, privacy, discrimination, harassment, and racial vilification;
(b) an infringement of intellectual property or copyright, whether of SBS or a third person;
(c) expressing prejudice against any person or group on the basis of their race, ethnicity, nationality, sex, age, sexual preference, religion, disability, mental illness, or marital, parental or occupational status;
(d) abusive, offensive, insulting, hateful, violent, threatening, harassing, vulgar, sexually explicit or obscene; or otherwise deliberately offensive or provocative towards other users on the Network;
(e) inappropriate, off-topic, or vexatious;
(f) an intrusion of privacy of any person, including the inappropriate provision of personal information or impersonating another person;
(g) an advertisement or promotion, including inappropriate or gratuitous references to a company or product which unduly promote that company or product; or
(h) a solicitation for donations.
5.5 In order for SBS to use your contribution, you confirm and warrant that it is your own original work, is not in breach of any law, whether the laws of Australia or another jurisdiction, that you have right to make the contribution and that you have the consent of anyone who is identifiable in your contribution.

User Participation. LICENSEE’s appropriate and authorized staff will have access to all LICENSOR Software Programs as set forth in Exhibit A.

1 - Only a subset of immunization providers have access to add or edit information 2 - Only a subset of users have access to clinic or provider level reports ▪ View Demographics & Immunizations indicates the user has permission to view information about the patient, including the patient’s name, date of birth, parent/guardian name, address, telephone number, the entire immunization history and status (i.e., whether or not the child is up-to-date with recommended immunizations). ▪ Add/Edit Information indicates the user can add new demographic and immunization data to a patient’s record; edit demographic and immunization data previously recorded in a patient’s record; and add a new patient to the KIDS Plus IIS. ▪ Clinic and Provide level reports indicates select users have access to run two levels of reports:

User Participation. Eligibility to participate requires that the Grantee agrees to maintain the QVF and update election and voter history data by:
1. Ensuring the entry of new and updated voter information prior to each election;
2. Ensuring all survey information is provided to the County as required for E-Wizard stages 1 and 2 within set deadlines;
3. Ensuring storage, distribution, and collection of the laptop/accessories, including installation and initial setup of the EPB on the laptops prior to and after Election Day;
4. Ensuring the entry of voter history within 14 days after election;
5. Evaluating and maintaining their street indexes.
6. Setting up ballot styles, AV counting boards and combined precincts when appropriate in the QVF software for every election.
7. Otherwise maintaining the QVF in any way deemed necessary by law or by the Department. The Grantee also agrees to:
1. Certify in writing prior to every election that the jurisdiction is prepared to use the EPB.
2. Attend and successfully complete a training session sponsored by the Bureau of Elections. The Grantee also understands that:
1. The EPB software contains personal voter information that must be protected;
2. All State data security requirements and procedures must be followed as identified in (Attachment 1). The Grantee further agrees that:

One or more receiving board(s) will be authorized and used in conjunction with the EPB and that the receiving boards will be responsible for reviewing and attesting to the genuineness of the reports generated from the EPB as supplements to the official Poll Book for each precinct and for performing other duties as defined to ensure the timely completion, assembly, and security of all required election documents and materials as defined by law. ▇▇▇▇▇▇▇ agrees to establish receiving boards for each election or otherwise comply with procedures prescribed by the Bureau of Elections. Every individual who wishes to participate as a user of the Registry must read this Organization Security and Confidentiality Agreement and sign and comply with the Registry User Security and Confidentiality Agreement (DPH F-00103). The Department administers and controls access to the Registry and the data contained therein. The Registry Organization Security and Confidentiality Agreement must be signed by a representative of the Organization prior to any training of Organization staff on use of the Registry or Organization access to the Registry data. This person shall serve as the Organization’s Site Administrator, and shall have responsibility for all individual users within the Organization. The Site Administrator shall complete the Department administered training for Registry Site Administrator. Having completed the training, the Site Administrator may enroll users who have been trained in the use of the Registry at the appropriate access level and have signed the Registry User Security and Confidentiality Agreement. The Site Administrator shall maintain a file of signed Registry User Security and Confidentiality Agreements and ensure that each user within the Organization renews his or her agreement every two years. Users with expired agreements or revoked privileges shall not access the Registry. The Site Administrator shall be responsible for each user within the Organization. Only personnel whose assigned duties include functions associated with the blood lead testing of children may be given access to Registry information. All such personnel, including permanent and temporary employees, volunteers, contractors, and consultants shall be required to sign a Registry User Security and Confidentiality Agreement before gaining access to the Registry. Whenever a user terminates his or her working relationship within the Organization, the user’s account shall be immediately deactivated by the Organization’s Site Administrator.
