Waiver of Insurance Requirements. The purpose of this waiver form is to provide a medium by which administrators can explain the reasons for a particular Contractor not being required to provide insurance according to the insurance provisions in Section 11 of this Agreement. This waiver form is to be used only in conjunction with the PTSA. Date: enter date Site: enter cost center and site name Contractor’s Name: enter contractors name Reason why administrator feels insurance requirements should be waived: Contractor will not be working with minor children. Contractor will be escorted by Xxxx County School Board employee during the entire time he/she is in the presence of minor children.
