The following terms are an addendum to your contract. WAIVER OF LIABILITY AND HOLD HARMLESS ADDENDUM FOR COVID-19 In addition to the other provisions in this contract, Contractor understands and accepts the following conditions: The novel coronavirus (“COVID-19”), has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies have provided orders, regulations, and guidance regarding COVID-19.

Wavier of Liability. Participants who have not completed the form will not be permitted to participate in camp/clinic activities until it is received. PARENT/GUARDIAN AGREEMENT I agree to allow my child/xxxx to participate in the Program/Camp/Clinic and affirm that my child’s/xxxx’x participation is completely voluntary. I understand that there are risks inherent in the activities my child will engage in which may cause serious injury or even death.
